Patient Forms

As part of our commitment to providing superior care, we've made critical new patient information available via a secured website.

You should have received a welcome email to the email provided to us in the New Patient call with a link to your secure account access. To retrieve your New Patient questionnaire:

  • Search your inbox for an email from anywheredolphin@dolphinimaging.com. If the email has a hyperlink to www.anywheredolphin.com, click on it. Your browser should take you to the login page and pre-fill your username.

  • If that doesn’t get you what you need, search your inbox for "www.anywheredolphin.com". You may have received an email directly from our office. It may also contain a link that will take you to the login page.

  • If neither of these helped, please give us a call at (281) 251-9229 or email us so we can resend your username and access information.

    Once you are logged in, you'll be able to view future appointments, appointment history, financials, images and questionnaire information for everyone in the family!

    If you have already successfully created a login and a password, please click here to access your account. You will need the assigned username and previously created password to login to the site as needed.
